內容大鋼：

一. 簡明 Functional smile design 報告
a. always start at TMJ-要先有mounting cast
b. contour design:
i. 先決定lower incisal edge的position(咬合器上咬到 後牙沒有接觸)
ii. incisal plane
iii. lower incisal 要有definite line angle及stable的contact
iv. lip-closure path 要prepare成兩個plane
v. (upper)incisal plane要 follow lower lip line
c. five steps to Harmony
i. 建立CR於前牙.後牙有干擾要修掉.前牙沒接觸要reshape
ii. long centric stop-pt坐90度輕咬得到VD(supine position)
iii. freedom from CR不會超過0.5mm.否則是錯誤的
iv. 前突時需要group function.
v. 側方運動時 ant.group function.或protective canine guidance
d.要達成前牙stability.comfort.function下列是key point:
in harmony with 1.lips
2.CR
3.phonetics
4.neutral zone
5.the envelope of function

二. 全口無牙全口重建時 先排下牙(決定vertical dimension-CR決定).lower incisal edge由retromolar pad前2/3和lower lip的中線決定

三.李XX(Dr.原)
46 implant failure懷疑是DM引起.也有可能是occlusal trauma.因為16有elongation.可能有overloading.若是DM可能不會那麼快鬆掉.且其他牙齒也可見peri狀況

17敲診會痛. 建議14-18 impression for TP.但suggest RPD或implant

六.implant.92015.92016需要拔牙同意書.有制式化的格式.也有麻醉同意書

十.光動力療法aPDT新進器械(二極體雷射) Trans.50mw.用於拔牙傷口和ulcer (一次30秒-660 5mw以上就會傷眼睛 要戴護目鏡).
